Manulife health and dental plan

The CAJ is pleased to offer its members a group health benefit plan through Manulife Financial.

Various levels of coverage are available to members in good standing who do not have coverage through their employer or may wish to supplement whatever coverage they already receive. The plan rates and coverage may vary from province to province, but this perq is available to all CAJ members across Canada.

Some details on the level of coverage:

Base plan

  • Dental services (not including orthodontics, crowns, bridges or dentures) – 70 per cent coverage up to $400 a year
  • Generic prescription drug coverage (not including birth control and/or fertility drugs) – 70 per cent coverage, maximum $6.50 dispensing fee, to a maximum of $750 a year
  • $100 for vision care every second year, plus $50 a year for optometrist visits
  • Accidental death and dismemberment, travel coverage and survivors’ benefits
  • Coverage for extended healthcare, homecare and nursing, prosthetics, medical equipment, hearing aids and ambulance services

Bronze plan

  • Dental services (not including orthodontics, crowns, bridges or dentures) – 70 per cent coverage up to $500 a year
  • Generic prescription drug coverage (not including birth control and/or fertility drugs) – maximum $6.50 dispensing fee, 70 per cent coverage of first $500 a year, 80 per cent coverage of next $2,500
  • $100 for vision care every second year, plus $50 a year for optometrist visits
  • Accidental death and dismemberment, travel coverage and survivors’ benefits
  • Coverage for extended healthcare, homecare and nursing, prosthetics, medical equipment, hearing aids and ambulance services

Silver plan

  • Dental services (not including orthodontics, crowns, bridges or dentures) – 80 per cent coverage up to $500 in the first year, $900 a year afterwards
  • Generic prescription drug coverage (not including birth control and/or fertility drugs) – maximum $7.50 dispensing fee, 70 per cent coverage of first $500 a year, 100 per cent coverage of next $4,650
  • $150 for vision care every second year, plus $50 a year for optometrist visits
  • Semi-private hospital accommodation
  • Accidental death and dismemberment, travel coverage and survivors’ benefits
  • Coverage for extended healthcare, homecare and nursing, prosthetics, medical equipment, hearing aids and ambulance services

Gold plan

  • Dental services (including some coverage for orthodontics, crowns, bridges or dentures) – 80 per cent coverage up to $750 in the first year, $1,000 in the second year, $1,200 in the third and fourth, $1,500 in the fifth year and beyond
  • Generic and brand-name prescription drug coverage (including birth control and/or fertility drugs) – including dispensing fee, 90 per cent coverage of first $2,222, 100 per cent coverage of next $8,000
  • $250 for vision care every second year, plus $50 a year for optometrist visits
  • Semi-private and private hospital accommodation
  • Accidental death and dismemberment, travel coverage and survivors’ benefits
  • Coverage for extended healthcare, homecare and nursing, prosthetics, medical equipment, hearing aids and ambulance services

Premiums range from approximately $41 a month to $365 a month depending on your age, whether you’re single or taking coverage for your family and where in Canada you live.
